Presentation of a rare case of synchronous primary pulmonary carcinomas with different histologies – large-cell neuroendocrine carcinoma (LCNEC) and sarcomatoid variant – located in two different lobes of the lung, along with a review of current therapeutic approaches. Systematic review of contemporary literature and analysis of diagnostic criteria according to NCCN guidelines and other scientific sources on similar cases. Synchronous primary pulmonary carcinomas with different histologies represent an extremely rare phenomenon, posing complex diagnostic and therapeutic challenges. Novel molecular data and innovative therapies, such as DLL3-targeted BiTE agents for LCNEC and first-line immunotherapy for sarcomatoid carcinoma, are revolutionizing the treatment of these aggressive diseases
